COMMON SITUATIONS
Real Support for Real Life
Need help staying organized, managing daily routines, getting around your community, handling stressful situations, or rebuilding skills after an injury or disability? That is what we are here for.
We meet people where they are, figure out what is getting in the way, and work with them step by step toward greater independence
What We Help With
More Independent Future
Independent Living Skills Training
Sometimes everyday tasks can become harder after an injury, disability, or major life change. That is where Independent Living Skills Training, or ILST, comes in. We work one-on-one with people to help them practice and improve the skills they need to be more independent.
- Keeping a daily routine
- Staying organized and managing appointments
- Planning meals and shopping
- Managing a household
- Using transportation and getting around the community
- Problem-solving and making everyday decisions
- Rebuilding skills that may have become harder over time
The goal is simple: help people do more for themselves, at their own pace
Empowering Positive Behavior & Student Success
Positive Behavioral Interventions & Supports
Behavior is often a way of communicating that something is not working. PBIS helps us look
beyond the behavior itself and understand what may be causing it.
We work with each person to identify patterns, triggers, and challenges, then build practical
strategies that can make everyday situations easier to manage
- Handle frustration and stress
- Improve communication
- Build healthier routines
- Respond differently in difficult situations
- Reduce behaviors that get in the way of personal goals
- Feel more successful at home and in the community
We are not just focused on stopping a behavior. We are focused on understanding what is
happening, finding better ways to respond, and helping the person move toward a more
independent life
Work With Us
What Working With Us Looks Like
We do not believe in a one-size-fits-all approach
Every person has different strengths, goals, routines, and challenges. So we start by getting to know the person. We look at what is going well, what is getting in the way, and what they want to improve. From there, we build a plan that makes sense for their real life
1
Listen
We learn what the person wants to work on and what matters most to them.
2
Understand
We look at the everyday situations, habits, skills, and challenges affecting their
independence
3
Build a Plan
We create practical goals and strategies that fit the person - not the other way around.
4
Practice & Adjust
We work on those goals in real life, track progress, and change the approach when needed.
Every Journey Begins Here
Small Steps Can Lead to Big Changes
Progress does not always happen overnight. Sometimes it looks like remembering an appointment without help. Sometimes it is learning how to take the bus independently. Sometimes it is staying calm during a situation that used to feel overwhelming.
Those wins matter. Our job is to help build on them
Who We Support
Home Care 7twenty6 works with people who may need extra support with daily living skills, behavior, independence, or adjusting to life after an injury or disability.
Our focus is helping people stay active in their homes and communities while building the skills and strategies they need to move forward
FOR FAMILIES
For Families and Support Team
When someone you care about needs support, it can be hard to know where to start. We keep the process simple. We listen, ask questions, learn about the person's goals, and help determine what type of support may be most useful. You do not need to know all the technical terms before calling us. Just tell us what is going on. We will help you understand the next step
FOR SERVICE
For Service Coordinators and Referral Partners
We work with service coordinators and community partners to make referrals as clear and straightforward as possible. Our team values communication, follow-through, and making sure everyone involved understands the goals of the service. If you have someone who may benefit from ILST or PBIS, reach out to our team
Our Goal
We want the people we support to feel more capable, more confident, and more in control of their lives
That is what independence means to us. Not doing everything alone. Having the skills, tools, and support to do more for yourself
